Note: The following questions about Instructional Design support do not affect the scoring/evaluation of your nomination, but the Online Steering Committee, which sponsors this award, is gathering data about how nominated courses are developed, which may affect future award processes.

Documentation

  1. Attach the end-of-course student evaluations in a single PDF, as posted to Watermark or HB 2504.

  1. Attach the Quality Course Design Rubric to which you have added comments that summarize how your course meets each of the criteria. Download the Rubric.

Ensure that you have followed all of the steps outlined in the Submission Directions section of the Award for Excellence in Online Teaching page. Nomination forms that do not include the required documentation (Quality Of Course Design Rubric and Student Feedback on Course Effectiveness Report) will not be considered.
